Here in the Boston area, we’ve had unnervingly little snow so far this winter — but the weathermedia are cheerfully predicting that we’ll double our seasonal tally by Tuesday. And right now, I have the barometric pressure headache to support that theory. … Braverman, a dog-sledder, author and correspondent for Outside magazine, is one of 17 women racing in this year’s Iditarod, a record 32.7 percent of the field.
“Mushing is one of the only sports where men and women compete together at elite level,” she says. “We are taken seriously as athletes because there’s no chance for people to tell themselves we’re not on the same playing field.”
But she doesn’t consider herself just an athlete. She’s also a coach, a nutritionist, a parent, even a veterinary tech for her team. All her dogs, 14 hand-picked racers from a group of 20 that she has been training, have undergone physicals as extensive as the preparations for any professional athlete in the NBA or NFL, from electrocardiograms to vaccinations.
The dogs are a sharp but motley crew of strong personalities. Pepe is the steady, “mature” head of the pack. Flame is Braverman’s “shadow” and has raced with her in every qualifier. Jenga, Flame’s half sister, “doesn’t suffer fools.”…
“People are getting to know these dogs as pets, as friends, and they’re also seeing them as elite athletes,” she says. “It’s like rooting for your favorite sports team, but they all happen to be dogs.”…

I have been in and out of 5 ERs since Monday. Every time up until Thursday I would just get turned back around home. I finally had to collapse in my apartment and my dad knocking my door down to get a hospital to pay attention. I have even more severe tingling in my legs than before and was twitching like mad. Am waiting on some images now.

Discharge was immediately cancelled and we actually started working through options. Now at a different hospital because they can do the specific imaging I need.

Okay, first off this movie will not be in everyone’s wheelhouse.
Worked my way through Zama and am at the end glad I did, albeit in maybe 20 minutes chunks as it is so languidly constructed that one gets antsy over things like extended shots of someone staring pensively into the distance. Still, such bits set the scene in beautifully shot locales, at once both mystical and ponderously mundane. As it is just about two hours long, took me several days to watch the entire film.
If you’re one who likes, say, Aguirre: The Wrath of God or the writings of Borges, consider this a recommendation. Available on Amazon Prime.

I really only lurk here (I’m not quick-witted enough to keep up with the conversations), but since it’s an open thread, maybe I can ask some advice of the southwest Ohio jackals?
My husband and I are taking a long weekend to the Cincinnati area soon. I’ll be on my own most of Friday afternoon while he takes a manufacturing plant tour, but we’ll have Friday and Saturday to explore, and drive home Sunday. I have a list of local attractions from the visitors’ bureau so far–anything you’d recommend, or suggest we steer clear of? My first preference is the zoo and botanical gardens, but it’s early March, so maybe that’s another visit, or if we do decide to relocate here.

Do you like it there? I’m trying not to get ahead of myself, but any advice is welcome at this point.

It does depend though. They would arrest you on a local warrant, but not on an out of area (misdemeanor or low level felony) warrant because if they do arrest him they have to transport him to where the warrant originated or get police from that place to pick him up, which they generally don’t do. So the ordinary expectation would be “Portland city or county warrant, arrest, out of area, won’t arrest”. If the BLM protesters were local with local warrants they were more likely to be arrested on an existing warrant.

I read that as “if we arrest you we will run you and notify the jurisdiction the existing warrant comes out of” – then they decide who gets him. Indiana, Michigan and Ohio have this whole seamless process that is partly dependent on if a judge wants them brought back or if they just hand off to the state where they were picked up on the new charge. Sometimes the new place doesn’t want them so they’re actually arguing over who has to prosecute them :)

You’re right. I cut myself grabbing a food processor blade. It wasn’t bad, but when it hadn’t stopped after 24 hours, I took myself to the ER. After telling them I was on blood thinners, I went right to the head of the line. Every other time I’ve gone to the ER, I’ve had to wait for hours.

@debbie: I’d like to say it was 100% effective but my last ER visit was the exception to the rule. Cut myself to the bone on a piece of stovepipe at a store. By the time i got to the cashier there was blood all over my hand and half my pants (the poor girl went googly eyed and could only stutter when I asked if the had a first aid kit) (they didn’t) When I got to the ER with a bunch of blood soaked paper towels taped to my hand they barely even noticed me. 3 hrs later I was still pacing the waiting room with an ever angrier face. My “agitation” was such that when they finally came for me they sent a security guard to escort me back.
Never did see a Doc. The NP who sewed me up asked, “You wouldn’t even be here if you weren’t on blood thinners, would you?”

They rip open the bags of recycling you’ve put out in the designated recycling bins in order to get the bottles that have deposits on them, then often leave the remainder on the ground (which the garbagemen won’t pick up), or dump them in the paper recycling bin (which means that you have to re-sort everything, and the garbagemen won’t pick the paper bin up if you don’t re-sort it before they arrive) or the regular garbage bin (which, again, means you have to pull them out and put them back in the glass/metal bin, or they won’t get recycled.) The city has had to remove all of the public recycling bins that used to be on street corners because the recycling thieves leave the bottles they don’t want strewn all over the street.

Mostly, they make sure you are not in imminent danger and try to address acute problems. The best outcome for most people is an educated referral to a specialist. I have a horror story about a family friend that even I had trouble believing that finally made me understand that.

We’re a blue city proper now, and with black flight to places like Sharonville, Northgate and assorted areas, even some suburbs are getting a blue tinge. There’s a westside/eastside division based on the fact that Downtown sits in the middle of the two sides of a large bowl.
Some advice on neighborhoods may be badly outdated. Many of the old inner-city neighborhoods are gentrifying (unfortunately for some of us) and may have new/remodeled construction to offer. Look around.
Bonomoni’s bakery is the best (Fah, Graeters and Servattis).
If you are coming from the coasts, you will find Cinci real estate remarkably inexpensive, unless you move into one of the newer built condos-and even many of those aren’t too bad. Middle Class people can afford a house here.

His eyes popped out of his head at the prices, mere fractions of what similar places would go for on the east coast. He brought the book back home with him and showed it to everyone he knew. It was the eighth wonder of the world to him.

When my neighbors moved here from Michigan and the husband, a family practitioner, had just started seeing patients, a significant amount of the people he saw were there to check out whether the new doc in town would write them painkiller prescriptions. He wasn’t surprised, apparently it’s well known that comes with starting a new practice. Eventually the word gets out that you’re a party pooper and they leave you alone.
Everyone needs a long-standing relationship with a smart primary care physician who is generous with writing referrals to specialists. At least that is my MO. Let the specialist rule out whatever it is I’ve made a preliminary diagnosis of (with the help of google). Most of the various dx’s the Ohio Family has were identified this way.

Yutsano, if you don’t already have one, call the number on the back of your health insurance card and get a PCP (Primary Care Physician). Great for initial exams, but the best part are the nurses who can help steer you through referrals and paperwork. And by the way, you may also want to get some FMLA paperwork ready. Whatever it is you are having, you may have several doctors appointments (specialists and pcp). FMLA also allows you to get advanced sick leave and even advanced annual). Also with a PCP, you may also have a nurse who can steer you through treatment issues, give you advice on whether or not to actually go to the hospital, refer you to a pharmacist and other options).

The IRS also have their forms too-ask your manager, and also ask for a printout for accumulated leave as well.

@Ohio Mom: I found ERs take chest pain very seriously, and it turned out to be the best place to go because they had equipment my primary care doc didn’t have (especially not at 4am). Most relevantly, they got blood test results within an hour and so knew for sure I was having a heart attack.
I think acute care facilities take care of some stuff now but I don’t know what kind of hours they have.

If the weather’s decent you might visit Spring Grove Arboretum – fair warning in case it would creep you out: while over 700 acres, it’s part of a cemetery. I have never encountered a clown in my many visits, however.

As for living here – my views are complicated because I was raised here and ultimately returned/stayed for reasons that aren’t entirely clear. I spent a good deal of my adolescence and glory days traveling to many other places, which no doubt helps. So listen to some of the jackals who had to move here for a better perspective.
There are a variety of types of places to live here, at many price points; the most interesting of which tend to be at the higher end, though not all are.
Plus there’s a week to arrange a meet up if you’re game to get together with local jackals, since we’ll have a variety of local views. Also, too, Blue Ash is a real place.

Getting help when you wake up in the morning and can’t walk without assistance because of severe pain should not require a grand strategy. And I will say when something similar happened to my brother they did not let him just leave but made sure he made it to a place where he would be further evaluated.

@Barbara: The PCP in this scenario isn’t for the acute stuff: call 911 or a ride to the hospital for that. But after the initial emergency workup there needs to be a focal point for you as a whole-rest of your health, keeping track of meds, referrals to specialists and therapists. In any event, the more, the merrier in terms of what is going on. I mean, going to the specialist for a cold or some condition that aggravates whatever it is you have?

And to be fair, it is sometimes hard for us non-medical people to correctly identify what is an escalating emergency — though some are obvious, such as severe trauma, chest pain, trouble breathing.
If you’re not sure, you can always call your PCP and whoever is on call will tell you whether your circumstances merit the emergency room. We’ve done that a few times.

ERs are for fixing immediate problems. It’s not the seriousness of the problem it’s who is closest to dying and can we do anything about fixing that. For some there is also a bit of how much will this cost and can we recoup some of that. But while this issue may be emergent it is a problem that will take an in depth look with equipment that they probably don’t have and time they can’t spend. If you read the thread here, notice that bleeding is a major indicator of the ER issue. Cut an artery and they will help you soonest. Something like this or having a major life threatening infection from a procedure at that same hospital doesn’t rate anything special. We’d like to think it does but that’s just from the victims side.
Also notice that I’m not saying this is right, just this is the way an ER works. I know I’ve spend enough time in ER waiting rooms to know. I once spent 1 1/2 hr in an ER waiting room and I was the only patient in the entire ER, not just the waiting room. They took an Xray and sent me on my way. $3500 total bill and my total interaction other than waiting was 10 minutes. One of the bills was for one dollar. For someone I never saw.

Given his resume, I was expecting Cleveland, Detroit, Pittsburgh; and there may be a good match back home through a headhunter. He applied jokingly to a place that could be a dream job for my bourbon nerd, and they called him back that day! So things got real very quickly.
I think I have the basis for a pretty nice trip, and I guess I’ll just have to see what next weekend brings.

@CarolDuhart2: Yes, say you have a serious lung condition, contacting your pulmonologist (with whom you have a longstanding relationship) when you have a cold may be the thing to do, even as the rest of us should just stay home and eat chicken soup.
It seems to me this overall discussion is confused because we (not just you and me, everyone chiming in) are talking about very different things. Some things are emergencies, and sudden severe symptoms are red flags.
If the ER disagrees with you and deems your symptoms not an emergency, you still need to follow-up on your own, and the place to start is with your PCP. If your PCP is not helpful, maybe you need a new PCP. No one is going to convince me there is no place for a PCP.

So what did it take?
Thing the first: me falling in my apartment unconscious and unable to be aroused. My dad had to call an ambulance and break down my door. They couldn’t wake me until a nurse was thumping my chest hard. They took a few tests and diagnosed rhabdomyolysis. I was feeling better a couple of days after that but the other issue I was having which was increased weakness and numbness wasn’t. I put my foot down. I said in no uncertain terms am I leaving this hospital until I know what’s wrong with me. The problem is they need to do an MRI and I can’t have that because of my spinal cord stimulator.
Then an absolute GENIUS of a spinal surgeon came up with an idea: amyelogram. But that couldn’t be done at the facility I was at so I hit up yet another hospital. Now waiting on the test

They do this differently in many parts of Europe and it works better. Not as excessive lengths of sentence, more humane treatment of prisoners, etc, etc and while you still have repeat offenders the goal is to end the shitty path, not extend it. A lot of time it’s just opportunity, having some or none. If you have none you have to take what you can find and that’s usually not a legal situation. And most of us continue down any road that gives us something even approaching opportunity, even if it’s a shitty road not worth traveling. And once you’ve gone down that road long enough, you know the language, customs, pitfalls, accommodations, etc, no mater if that road is criminal, it becomes the only road one sees, especially if that is reenforced by the system that it’s immersed in. We need to introduce intersections in that road, not roadblocks. Intersections are opportunities to change, to grow, and yes to prosper. Not everyone will take them but if they don’t exist no one will.

My primary got me started down the cancer treatment road. I had no obvious symptoms, I had no idea. 4+ yrs later I’m in remission.
I’ve had 6 different primaries at the VA, everyone of them served a very valuable place in the health care system for me. None of them were doctors, they have been Nurse Practitioners or something to that effect. Your first line contact with the medical profession. I was told by my doc almost 50 yrs ago that the then old time family doc would be going away and we’d have screeners to direct us to specialists and take care of minor issues.
He was exactly right, this is the role that a primary plays.

@Brachiator: They are wrong. ER is its own discipline, dedicated to triage and stabilizing patients. Not at all the same as following patients with conditions you know about in great detail.
If you are a top doctor, you are not hanging around the ER with the random hodgepodge. You are in your office seeing a carefully curated series of interesting cases.

@Ruckus: Exactly. There have been a number of times I’ve shown my PCP something and she has shrugged and said, go (full in the blank).
Then I go wherever and that medical professional takes a quick look at says what it is and what we have to do. The next time I see my PCP, she is glad we have checked that off and we go on from there.
I don’t need my PCP to know everything, I only need her to take me seriously and have good specialists in her Rolodex.

@Brachiator: Are you kidding? First, it can actually be hard to find a pcp who is open to non-concierge patients. Second, if you are under a certain age there is no reason to think you need a doctor, until you do. Third, there are people who find it immensely difficult to justify taking time off from an hourly wage job just to get a check up and have a pcp on speed dial. Seriously, those are the top 3 and that is before we tackle language and other cultural barriers (what if you have to take the bus or don’t speak English?). The chasm between consumer need and industry self-perception is practically unbridgeable when it comes to non-life threatening medical problems that nonetheless need attention and follow up.

I have a grudge against ERs that send people home without a careful look and that is what this sounds like to me.
My dad at 94 was sent home three times and my sis was told to just keep him hydrated, that his diarrhea would pass soon enough, without listening to Dad telling them it had been over a week by the third visit. My sister is not very forceful and Dad was too weak to fight them. The fourth time he went in, about 24 hours later, his doctor happened to be there and had him admitted immediately. He died a few weeks later after surgery to repair a perforated colon that was the result of a cascading failure of almost every care giver over that period, including the hospital but not the surgeons or anyone after that, 4 days after the surgery. There was nothing else wrong with Dad. He was very healthy for a man his age: weight, BP, cholestrol, and everything else in the good range.

But.
And you knew there would be a but. The system has a few issues, more than any of us are going to cover. Actually you covered numerous systems that should interact but fail to do so reasonably. Not everyone is covered. Going to a doctor is often far more expensive than just the medical bills. Many people don’t have access or ability to pay the often extreme and also often lopsided costs. But we have laws about ER access because that is often the only access people have to medicine. An ER could be the thing that a lot of people think it is, first line access to the medical profession. That’s not what the medical profession thinks it is but that is what a lot of users think it is mainly for the reasons we’ve just covered. This country seems to consider healthcare as insurance. But of course insurance isn’t healthcare, it’s a way to pay for it. They really are two separate issues. Even if we as a country paid all the medical bills of everyone, completely out of taxes, you’d still have a supply problem. The healthcare system is built around the healthcare insurance business. Money is the dominate issue, not healthcare. That is a discussion we as a country need to have. But we won’t because money is the answer to every question here. Not need, not how, not even what or when, it’s always how do we pay for it first and foremost. And if that cost is seen as too much then no other questions are ever asked. Some of that is linked to the ridiculous question of “those people” and what they deserve.

No. I’m not kidding. And yeah, we can reel off all the supposed difficulties, but this still doesn’t make using the ER as your first choice primary care option reasonable.
In California, urgent care facilities have popped up all over the place despite the success of Covered California, Medi-Cal and other insurance options.
And again, my original observation was about people who probably can easily find a primary care physician who think that tying up ER personnel gets them first rate medical care. Some of these people are also in good financial shape, but slow pay or skip out on emergency room billing, easier to do when billing comes after treatment.

I was just a bit too young to be a hippie, and for decades I really regretted that. I loved what seemed to be the hippie credo, love and peace and brotherhood.
But over the last many years, I’ve been rethinking that – mostly because it’s obvious to me that the right wing resurgence comes from much the same anti-authority impulse. It has occurred to me that anti-authoritarianism is a kind of reflex, and the actual merits of opposing or supporting a particular authority are glosses that get retrofitted onto “the movement.”
I now don’t think the “old hippies” were so much anti-war as they were anti-authority. Authority happened to be telling them to go fight a war, so they were against the war. Authority was also telling them to grow up, get a job, and be like their parents. So they were against that, too – not necessarily for any high-minded reasons, but because they didn’t want to be told what to do or what kind of life to have.
The high-minded reasons came later, courtesy of people who really had thought about it, and really did have philosophical foundations for their positions. But those were far and away the minority of the hippie movement. If you see what the vast majority of the hippies made of their surroundings and their lives, it doesn’t look very pretty. For every commune that survived and prospered, there must be five that were little more than white trash slums.
There are still some old hippies, and many of them live the lives they claimed to stand for: communitarian, loving, peaceful. But it seems to me an awful lot of also became survivalists, conspiracy nuts, and stand-your-ground types – and slid very smoothly into the Right Wing movement.

Good friend of ours who worked in the government side of health care recommended visiting an Urgent Care facility, so they could forward you to the ER in an ambulance, which gets you seen asap in the ER.
Yutsano, your symptoms sound like issues I have had with compressed nerve issues in the past. I still have minor problems, sometimes wake up at night with numb/painful hands, which sounds like a contradiction, but numbness due to nerve impingement is not pain free, quite the contrary.
My first event was nearly 40 years ago when my legs went numb in a kayak doing white water on New River. When I saw the family doctor Monday morning, he diagnosed the issue as muscle spasms in my lower back, causing the nerves to be pinched. Later on there was quite a bit of pain in my lower back as well.
Also sounds like you need an alert device you can use to call for help by pressing the bright red button. The Fire Department is qualified and authorized to break down doors and transport an immobile patient to the ER, and there too you get quick attention from the ER staff.

I know hippies aged from mid 70s to 19 y o. So you guys weren’t too late at all. Many of my hippy friends worked hard building houses, barns, bridges, sewers and water lines, etc. Many of us are disabled from the hard work. Others still milk goats and make organic cheeses.
And Faux Hippies who turned into right wing nut jobs aren’t any stranger than Boy Scouts who turn out that way. Same for Harvard graduates and dropouts!
